Privacy notice
This notice explains what information I collect, why I need it, who may see it and what your rights are.
Who is responsible for your information
I’m Alex Mouzakitis, a physiotherapist based in Northampton. I’m responsible for the personal information you give me when you enquire through this website, and when I treat you on a home visit.
If you have any questions about your information, email [email protected] or call 07356 247243.
If you’re seen at a Move4 Physio clinic, Move4 Physiotherapy Ltd also holds information about you. Move4 Physio’s privacy policy explains how.
What information I collect
- Contact details: your name, phone number, email address and, for home visits, your address.
- Health information: your symptoms, medical history, assessment findings, treatment and progress.
- Appointment and payment records: dates, fees and travel charges.
- What you send me: your WhatsApp messages and emails, and any notes I make of phone calls.
- For under-18s: the name and contact details of the parent or guardian, and their consent.
You don’t need to send detailed medical information in your first message. A brief outline is enough.
Why I use it, and the legal basis
| Why I use your information | Legal basis |
|---|---|
| Replying to your enquiry and arranging appointments | Taking steps at your request before a contract, or performing our contract |
| Assessing and treating you, and keeping clinical records | Performing our contract, and providing health care under a professional duty of confidentiality |
| Keeping financial records | Legal obligation |
| Responding to complaints or legal claims | My legitimate interest in handling them, and establishing or defending legal claims |
I don’t use your information for marketing, and I don’t make decisions about you by automated means.
If you don’t give me the information I need, I may not be able to treat you safely.
Who may see your information
I keep your information confidential. I only share it:
- with your GP or another health professional, when this is needed for your care and you’ve agreed, or in a situation where I’m allowed to share it without your agreement
- with a parent or guardian, where this is appropriate for a child’s care
- when the law requires it, for example with a safeguarding body, a court, a regulator or HMRC
- with my insurer or legal advisers, if there’s a complaint or claim
These services also handle information as part of how I work:
- WhatsApp (Meta), if you message me there
- Gmail (Google), if you email me
- Cloudflare, which hosts this website
These companies may process information outside the UK under their own safeguards. If you’d rather not use WhatsApp or email, please call me instead.
This website
This website has no contact form, adverts or tracking cookies. Like most websites, its hosting provider may process technical information such as your IP address to deliver the site and keep it secure.
How I keep it secure
I keep my clinical notes on a password-protected computer. Only I have access to them.
How long I keep it
- Adult clinical records: 8 years after your last treatment.
- Children’s clinical records: until the patient’s 25th birthday, or 26th if they were 17 when treatment ended.
- Enquiries that don’t lead to treatment: 12 months.
I may keep records for longer if they’re needed for a complaint or legal claim. Financial records are kept for as long as the law requires.
Your rights
You can ask me to:
- give you a copy of your information
- correct information that’s wrong
- delete your information, or limit how I use it
- transfer information you gave me to another provider
You can also object to how I use your information where I rely on legitimate interests.
Some of these rights have limits. For example, I may need to keep clinical records even if you ask me to delete them. To make a request, contact me using the details above. I’ll usually reply within one month, and there’s normally no charge.
Complaints
Please tell me first if you’re unhappy with how I’ve used your information, so I can try to put it right.
You can also complain to the Information Commissioner’s Office (ICO), the UK’s data protection regulator, on 0303 123 1113 or at ico.org.uk/make-a-complaint.
